Initiate an approval on a request (descriptor or policy).
Starts a new approval lifecycle on an existing REQUEST ticket. Pass
EITHER approval_descriptor (from getIgaResolvedEntitlements) OR
policy_id. Approvers and criteria come from the referenced policy —
you do not pass them in this call.
